Clinics & Services

The practice will provide essential services to its patients.

These are defined as:

• Patients who are ill or believe themselves to be ill, with conditions from which recovery is generally expected, including relevant health promotion advice and referral as appropriate, reflecting patient choice.
• General management of patients who are terminally ill.
• Management of chronic disease in the manner determined by the practice, in discussion with the patient.

The practice will also provide continuous holistic treatment and care for all registered patients.

In addition the practice will provide:

• Cervical screening
• Contraceptive services
• Vaccinations and immunisations (non-childhood)
• Childhood vaccinations and immunisations
• Child health surveillance
• Maternity services
• Asthma and COPD monitoring
• Diabetic Clinic

With effect from 1 February 2011 NHS Surrey has withdrawn funding for minor surgery services within general practice.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make the initial appointment as early as possible  - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travelto allow the vaccines to work.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting six days or less. Your employer may, however, require you to complete a self-certification form SC1 which is available from your employer.

For any illness lasting longer than six days you will need to see the doctor for him/her to issue a sickness certificate and for any subsequent renewal of the certificate.

Physical Health Services

Experienced paramedical specialists are available providing both NHS and private services. Private appointments are available to patients not registered at the practice. Please ask at reception for details.

Chiropractors are specialised in the diagnosis, treatment and prevention of disorders of the musculoskeletal system, particularly those involving the spine and their effect on the nervous system.

Community Dietitian - Mrs Nicola Smith
Dietetics clinics offer help in the management of diabetes, obesity, heart disease, allergies, high cholesterol and other diet-related conditions.

Counsellors
They are available on the NHS following referral from your GP.
