Flu season is upon us! Please book/ask for your flu jab from October
We are running online bookable and walk in flu clinics for adults on Saturdays 5th and 19th October.
If you are aged between 40 and 74 you are entitled to a free NHS health check every 5 years. If you would like a health check, please book online using Patient Access, or contact us by email or Dr IQ.
There are two steps to register online. We do require you to do both.
1. Click here to complete the online registration form. Please ensure you complete ALL sections including information about your medical history.
2. Click here to provide some essential further additional information required to complete your registration. We cannot approve your registration until this form has been received. Once completed, email the form to email@example.com.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form and new patient questionnaire, fill them out and send them/post to the practice/drop into the letter box. If you register on paper, please can you also copy a form of identification such as passport, driving licence or copy of other typescript document that gives your name and date of birth. This is to avoid spelling errors that can occur with handwritten documents.

If you live outside our catchment area, you may still be able to register with us. We will not be able to offer you home visits, but you will be able to access all other services.
All patients are allocated a named GP on registering at the practice, if you do not know who your named GP is please ask when next contacting the practice. This does not exclude you from seeing any GP of your choice.
